In any case, if you are using a CMS like WordPress the upload plugin will check that the file extension is an image type. Then you only need to ensure that users (apache) only have write permissions but not execute permissions on the upload folder.

As for filtering the image file that is uploaded, there are plugins that convert file formats, rescale and change resolution. For example an image can be 300 dpi which is too high a quality for computer screens which are only good for 96 dpi. Such high quality can also mean an excessively large file size and an unwanted overhead.

Any "image" file that survives conversion to 96 dpi will not survive if it is not a real image file. usually an error will occur that will solve your problem.
